Health Services

CSC of Eastern Hancock Health Services promotes the health of students, identifies and prevents health problems and injuries, provides proper care to students ill or injured at school, and in collaboration with parents and the student's primary care provider, develops and implements health care plans to ensure students are ready to learn.  School nurses strengthen and facilitate the educational process by improving and protecting the health status of children and by identification and assistance in the removal or modification of health related barriers to the learning process for individual children.

*Note all new Indiana resident students or those suspected of having a defect are screened.

Parental Authorization Forms for Medication Administration & Special Health Procedures

Each year Health Services asks parents to provide information about your child's health in order to help keep our student's healthy, safe and ready to learn. There are forms on this page that can facilitate this process. If your child's health requires health care while at school, we encourage collaboration between the student's health care provider and school nurse to develop your student's school health care plan.
